Integrated Care Services Collaborative Care Model

Our integrated care collaborative care model uses a team-based, interdisciplinary approach to deliver evidence-based diagnoses, treatment, and follow-up care.
Our team is led by a Primary Care Provider (PCP) treating patients with mental health/substance abuse conditions and is supported by a Behavioral Health Care Manager and a Psychiatric Consultant. The team implements a measurement-guided care plan based on evidence-based practice guidelines and focuses particular attention on patients not meeting their clinical goals.  The team collaborates weekly in regards to the provision of whole person care.
